How about a BIG one.?! The C5A that crashed at Dover AFB,DEl, last April. A buddy who flew corporate 737 and gulfstream described the CVR which he has. Three INSTRUCTOR PILOTS,SIM rated also. Lost engine on takeoff. Powered back, killed dead engine. Notified tower of return to field. Cleared. Descending out of 3500' PIC powered up two good engines and bad one he had just shut down. (Naturally it did not respond.) He's on two and slowing DOWN! NOT one of the THREE...PLUS instuctor FLIGHT ENGINEER caught mistake. Now slowing to 145. At 140mph, and 150 feet it stalled just off threshold....half mile, and crashed...100 G impact, broke into three pieces. No one killed due to immense size and crush factor but several badly injured. Amazing but not uncommon. (IE: PIC of Swissair near Halifax Nova Scotia...bad decisions killed all.) Old news dept: Mallard in Florida was found to have a cracked spar, severely corroded. Bad Maint.
