Annual inspection and AD/SB forms?

Anyone have a good checklist or form for doing an annual on an Aeronca? How about a AD/SB sheet?

We need to do both my Chief and Gareth's Sedan and was hoping that someone has ginned up a form that would make it easier.

I have one in "My Documents" that I can forward to you. I've used it for several years. Think I have to send it as a .jpg. Is that right??......Not too good on this computer business. I'll be gone for a day or so, but if you want a copy respond to my email address and I'll get it out to you.
